Applicant Instructions

Pre-K 3

• Submit an application with a $75.00 non-refundable application fee
• Children must be toilet trained and must be 3 years old by August 1
• Submit the Parent/Guardian Questionnaire
• Please note: For those families wishing to matriculate to Pre-K 4 the following year, we take developmental readiness into consideration. If your child is currently attending our Pre-K 3 program, all developmental readiness observations and assessments required for Pre-K 4 placement will be completed before February during school hours while your child is in attendance. Matriculation and Facilities Fees will apply for Pre-K 4.

Pre-K 4 – Grade 1

• Submit an application with a $75.00 non-refundable application fee
• For Pre-K 4 and Kindergarten applicants, submit the Parent/Guardian Questionnaire
• For Grade 1 applicants, submit the Family Survey and provide a copy of the student’s most recent Kindergarten report card
• Provide the Pre-K 4 - Grade 1 Teacher Recommendation Form to your child's teacher for completion
• Submit the Records Release Form signed by a parent

Grade 2 - Grade 3

• Submit an application with a $75.00 non-refundable application fee
• Provide a copy of the student's most recent report card
• Provide a copy of the student's previous year's report card
• Submit the Family Survey
• Provide the Grades 2-3 Teacher Recommendation Form to your child's teacher for completion
• Submit the Records Release Form signed by a parent

Grade 4 - Grade 11

• Submit an application with a $75.00 non-refundable application fee
• Provide a copy of the student's most recent report card
• Provide a copy of the student's previous year's report card
• Provide copies of the student's most recent standardized test scores (reporting in stanines and/or percentiles only)
• Submit the Family Survey
• Provide the recommendation form to your child's math and English teachers for completion (Grades 4-6 Teacher Recommendation Form) or (Grades 7-12 Teacher Recommendation Form)
• Submit the Records Release Form signed by a parent
• Applicant must complete and submit the student questionnaire and essay (Questionnaire for Grades 4-6) or (Questionnaire for Grades 7-12)

Once the student enrolls at Canterbury, the following required documents are due prior to attending classes:

• A copy of the child's birth certificate
• If the child is not an American citizen, please provide a copy of passport or visa
• Blue Florida Certificate of Immunization (form 680)
• Gold School Entry Health Exam (form 3040)
