The Florida High School Athletic Association and Canterbury School require students participating in team athletics to complete two forms: Consent and Release from Liability Certificate (2 pages) and Preparticipation Physical Evaluation (2 pages). You may download these PDF documents, complete them and return them to Lee Peters, Athletic Director. 

Any athlete wanting to play for a college Division I or II team must apply through the NCAA Clearinghouse >>>
